Directions for Registration: Quadrant Biosciences

The Westchester County Schools COVID-19 Screening Testing Program offers FREE saliva-based COVID-19 testing to students, teachers, and staff. Testing is quick and painless. The goal of the program is to reduce the spread of COVID-19 in schools so they can remain SAFE and OPEN for learning. The more students that opt in to the program, the more effective it will be!

In order for your student(s) to participate in the Westchester County Schools COVID-19 Screening Testing Program please take the following steps:

  1. Complete the ‘Consent for PCR Saliva Testing: Westchester County Schools COVID-19 Screening Testing Program’ form. Depending on your child’s school, this may be available electronically and/or on paper.
  2. Complete the ‘Authorization to disclose protected health information (COVID Test Results) form. Depending on your child’s school, this may be available electronically and/or on paper.
  3. Create an account with on the Quadrant registration and results portal (instructions below)
  4. Create a profile on the Quadrant portal for each of the students in your household (instructions below)

The estimated time to complete all of the steps above is less than 10 minutes. Once you complete these steps one time, your student(s) will remain enrolled for the rest of the program unless you revoke your consent in writing.

How to create an account with Quadrant

You only need to create one account regardless of how many students you have in your household.

Estimated completion time: 1 minute

  1. Go to Clarifi COVID-19 (clarifi-covid-19.com)
  2. Enter your (parent/guardian) email address
  3. Create a password
  4. Re-enter the password to confirm it
  5. Check your email for the automated verification code and enter it.

How to create a profile for your student(s)

Create one profile for each of the students in your household. You can create multiple student profiles under one Quadrant account.

Estimated completion time: 3 minutes

  1. Organization

Organization Type: K-12 School

County: Westchester

School: Choose your student’s school from the drop down list

Click ‘Verify’

Click ‘Next’

  1. Personal Information

Name: Enter your student’s legal first and last name

Gender: Enter your student’s gender

Date of Birth: Enter your student’s date of birth

Race: Enter your student’s race

  1. ‘Other’, ‘Decline to Answer’, and ‘Unknown’ are all options

Ethnicity: Enter your student’s ethnicity

  1. ‘Other’, ‘Decline to Answer’, and ‘Unknown’ are all options

Click ‘Next’

  1. Current Address

Phone Number: Enter your (parent/guardian) phone number with no dashes, spaces or parentheses

Address: Enter the address where your student is currently living

Click ‘Next’

  1. Legal Address

Address: Enter your student’s permanent legal address. If the same as above, click ‘Fill-in’ from Current Address

Click ‘Next’

  1. Student/Employer Information

Employment Status: Select Student – Full Time

Student/ Organization ID – if you know your student’s student ID number, you may enter it. If not, leave blank.

Employer Name: Leave blank

Address: Leave blank

Click ‘Next’

  1. Insurance Information

Does the patient have insurance?: Select No

Skip all remaining Insurance fields

Click ‘Next’

  1. Test Authorization

Read each of the three statement and check the boxes in order to consent

Click ‘Submit’

Repeat for each student in your household.

You are done! Anytime your student(s) receives a test, you will get an email alerting you when their results are ready. You can view the results by logging into the Quadrant platform. If you have any questions about when your student(s) will receive testing, please reach out to your student’s school.
