Health Screenings

Daily health Screening

KLSD believes we have a shared responsibility to minimize the spread of the coronavirus. For this reason, all staff and students are directed to stay home if/when they are exhibiting symptoms of COVID-19. Additionally, all students and staff are required to conduct daily health screeners in order to determine whether they are able to report to school/work.

For Students/Families:

Keeping your child(ren) and all the students and staff at KLSD safe and healthy is our highest priority. It is imperative that you keep your child(ren) home if they are sick in order for us to keep our schools open.

The following screener will be provided to families electronically on a daily basis. It must be completed and submitted daily. When submitted, the school will be notified only as to whether the child is cleared to report to school or not. Children who are not cleared, will be contacted by the school if the family has not contacted the school first.

If your child is showing any of the symptoms noted below and/or if you answer yes to any of the questions, you must keep your child home and notify our school nurse immediately at the number listed on your school’s website. The nurse will discuss whether it is safe for your child to attend school.

In the past 24 hours, has your child experienced any of the following which they were not experiencing previously, and which are not related to a pre-existing condition?

  • Fever or chills
  • New or worsening cough
  • Muscle aches
  • Fatigue
  • New loss of taste or smell
  • Shortness of breath or difficulty breathing
  • Sore throat
  • Congestion or runny nose
  • Nausea or vomiting
  • Diarrhea
  • Headache
  • In the past 14 days, has your child:

Experienced any symptoms of COVID-19 listed above, including a temperature of greater than 100.0°F in the past 14 days?

  • Yes
  • No

Knowingly been in close or proximate contact in the past 14 days with anyone who has tested positive through a diagnostic test for COVID-19 or who has or had symptoms of COVID-19?

  • Yes
  • No

Traveled internationally or from a state with widespread community transmission of COVID-19 per the New York State Travel Advisory in the past 14 days?

  • Yes
  • No

Finally, please take your child’s temperature prior to the start of each school day. If the temperature is 100 degrees or higher, please keep your child home and report this information to our nurse.

If you have checked any of the symptom boxes above or answered “yes” to any of the questions, please keep your child home and contact your school nurse immediately.

If your child is already in school when the school learns of a positive response to any of the questions above, your child will be directed to the isolation area located near the nurse’s office and the family will be contacted for immediate pick-up.

For Staff:

All staff, on a daily basis, will be required to complete the same screener and questions shown above. The information will be submitted by use of an electronic portal. Any staff exhibiting symptoms or who answer yes to any of the questions must stay home and contact the school nurse and/or administration to report and discuss their absence. If necessary, the school will consult with the District’s doctor to determine appropriate next steps.

For Visitors, Guests, Contractors, and Vendors:

At this time, KLSD is minimizing the access of visitors, guests, contractors, and vendors to our schools to the greatest extent possible. Any person visiting the school should do so only by appointment and must first be seen by the school greeter prior to entering the building. The greeter, in consultation with school administration and/or designee, will determine if access to the building is appropriate.

If access is appropriate, the visitor will be provided a hard copy of the same screener questions shown above. If the visitor answers yes to any of the screener questions, the visitor will be denied access to the building.

Additionally, any visitors who are allowed access to our buildings must wear a face covering at all times.
